Here’s a weekly-progress-report template I created and used with both students and parents last year. This template was especially designed for students with problematic performance, because I needed to record and monitor the student’s specific tactics and behaviour in every class. At the end of the week I kept a copy and handed the original to the parent or the student. This way I could remember and discuss everything I had taken down with the student, the counselor and the parent. In addition  parents and children had the opportunity to virtually go over the student’s activity in the classroom. 

The template provides a double table for either a boy or a girl student and it could be photocopied on both sides of a paper sheet. The class dates are to be noted  in the narrow column on the left, while the actual student performance is to be described under each star (for good practices) or under each figure (for practices that have to change). I preferred to write my comments in the first person singular since they were intended to be read by the student and have them involved. Comments could read: “I handed in my homework”, “I was concentrated during the listening task”, “I spoke impolitely to -X- and she was hurt”, “I refused to complete exercise 1”, etc. The name of the student can be written at the top, while there is space for the teacher’s and parent’s signature at the bottom. 

I think this procedure helped everyone involved and contributed to outlining successful and problematic practices.
